1. Book the Program Registration Package by selecting a MOA Icon.

2. Select Program Registration Package service.

3. Select an available 8:00am appointment from the calendar. This is not an appointment; this will take you to the mandatory registration/intake form.

4. Upon completing the online registration form and submitting the mandatory user manual to the PG Division moanetwork@pgdivision.ca, you can access the Locum MOA Service of your choosing and book according to your practice needs.

5. Click on the Locum MOA service you would like to book and then select the dates required.
